Frequently Asked Questions

How do I make new patient appointment?

1. My office manager, Cheri, does a phone intake on all potential new patients. She will get a brief history, demographics, and insurance information.  Cheri works about 10-12 hours per week, often from home. She is generally in the office only on Wednesday and Friday mornings.  Cheri gets phone messages and emails most days.  You can leave her a phone message or email message at any time.

2. Cheri will mail you a paperwork packet with a postage paid envelope. She can also email the packet. You will need to complete paperwork and return it to our office prior to scheduling a first appointment. 

3. Once we receive your completed paperwork, she will schedule an initial appointment. She can schedule either over the phone or via email.

Do you do emergeny or urgent new evaluations?

No, I do not have capacity to see new people urgently. There is generally a 4-6 week wait for a new apppointment. Sometimes that time frame is shorter or longer depending on the availablity in the schedule. Urgent appointments can be arranged for establisted patients only.
